You will need to fill out an intake form before your appointment. *Click here for the form. If you print them out at home be sure to bring them with you. If you forget your forms you will have to fill them out again before the appointment. If you need to fill them out in our office come 15-20 minutes early to ensure you maximize your time in therapy.

Our center is a chemically sensitive environment.  Please do not wear any cologne, perfume, heavy oils or smoke before your appointment.

We have a 24 hour cancellation policy.  If you cancel or reschedule your appointment during the 24 hour period prior to your appointment you will be charged full price for the appointment.
